An Operational Chiropractic Perspective
A single knob proportionally adjusts both force and depth of the thrust. A momentary pull of the trigger initiates a single thrust, while holding the trigger down allows continued rapid thrusting until trigger is released. When a chiropractor and a thrusting device team up to deliver a shallow, safe, optimum dynamic thrust, the factor that can compromise this thrust is a soft tissue variable. Before thrusting, take the slack out of the soft tissue. This stretches and firms the soft tissue to a density that can support and communicate the full dynamics of the shallow thrust to the joint. The recommended safe depth and force settings given in the instruction manual assume the joint has ideal soft tissue. Typically, the recommended soft tissue displacement prestress counter balances the spring force against the force limiting knob. This makes the prestress force directly proportional to the thrusting force. It's then quantifiable and reproducible. It's this simple: just set the force limiting knob as recommended for the articulation and press the ArthroStim™ Instrument against the patient until the limiting knob raises very slightly. Then begin thrusting.

The maximum stylus travel forward is 5mm, however, recoil travel is 10mm. This unusual recoil completely releases and stretches soft tissue with each thrust, effectively allowing the bone to find home. Chiropractors should not substantially override the optimum recommended depth and force settings of the force limiting knob in their effort to accommodate extraordinary soft tissue variables. It's usually unnecessary to override recommended settings to accommodate extraordinary soft tissue, especially when the Tissue Displacement Torque Sleeve is used effectively. Simply slide this accessory sleeve over the stylus. Then position the ArthroStim™ Instrument over the articulation, using your free hand to take the slack out of the soft tissue by holding moderate static finger pressure on the rubber grip around the sleeve. If the joint requires substantial preload or prestress similarly, use the sleeve and the free hand to do so. Additionally, when adaptive torque on the joint must be removed, apply pressure to the sleeve, then simultaneously rapid thrust and rotate the sleeve a quarter turn in the appropriate direction. A shallow dynamic thrust is minimally compromised by this sleeve because the stylus is free to oscillate or optimally thrust within the loose fitting sleeve even when it is rotated. (Caution: Don't attempt to displace all soft tissue and bottom out the stylus on the bone as this can damage soft tissue. The patient must have a soft tissue cushion and bone structure sufficient to withstand the thrusting without damage. If a doctor were to ignore safety and hold the ArthroStim™ Instrument rigid against the patient, the maximum impact could exceed 40 lbs.)

The spring balanced floating stylus of the ArthroStim™ Instrument provides a skilled chiropractor with feedback as to what extent he or she is overriding the recommended optimum mechanical thrust. For example, suppose while positioning the ArthroStim™ Instrument on a joint, a chiropractor attempts to accommodate an extraordinary soft tissue variable by pushing the stylus into the soft tissue. In doing so, he or she establishes what is believed to be a soft tissue density that will support and communicate the full dynamics of the thrust to the joint. Additionally, suppose that this prestress raises the force limiting knob so a visible 2mm gap shows between the knob and the spring. This indicates that the thrust deemed appropriate for the patient is measurably greater than originally intended or recommended. The experienced chiropractor then decides whether to pull the trigger to follow through with override, change the force setting, or use an accessory sleeve to take out the extraordinary soft tissue slack.

In practice doctors quickly get the feel of the recommended prestresses and thrusting forces and thereafter each adjustment becomes automatic, intuitive, fast and effortless. Only the ArthroStim™ Instrument provides this feedback. A chiropractor soon recognizes that a floating stylus provides other useful feedback. Initially, when the ArthroStim™ Instrument starts rapid thrusting into a patient, there are equal and opposite soft tissue resilient forces that momentarily stabilize the floating stylus. As the soft tissue and the joint comply, the chiropractor often hears or feels these opposing forces shift. The sensation is most noticeable when the soft tissue prestress counter- balances the appropriate force setting as recommended and described above. This equal and opposite soft tissue reaction is Newton's Third Law of Physics. This physical law also makes it impossible for a chiropractor's posture and grip not to affect the thrust of a hand held instrument.

The chiropractor must be aware of the fact that as an instrument operator, he or she not only controls the instrument's thrusting force, but also other forces that supplement its mechanical thrust. With that in mind, it is most advantageous that a chiropractor understands, utilize, and control the affect he or she has on the thrust. A chiropractor should understand that holding an instrument with a limp or a rigid grip affects the thrust. When a chiropractor's arm and grip are intentionally rigid, they absorb less instrument thrust, resulting in a greater thrust being delivered toward the patient. Likewise, when a chiropractor forces the instrument into the patient, he or she supplements the thrust. The chiropractor is not the one being adjusted, so by design the ArthroStim™ Instrument gives the chiropractor a mechanical advantage, which protects the chiropractor from stress and strain by minimizing the recoil or kick felt by the chiropractor, so his or her perception of the thrusting force is relative, not actual.

A chiropractor should experience as a patient the thrust the ArthroStim™ Instrument produces at various settings on various articulations. Initially it is recommended that the chiropractor try the force setting against his or her hand prior to adjusting the patient to insure that the setting is correct and that a previous setting is not inadvertently being applied. The manufacturer intends a limp, effortless grip on the instrument. Chiropractors may find it initially hard to believe that the recommended shallow dynamic thrusts can effectively clear an inappropriate adaptation or stimulate appropriate adaptation, so their tendency is to use greater force than necessary for effective results. Important: At first glance the recommended settings appear to be low, and that is misleading. At the recommended thrust setting the ArthroStim™ Instrument inputs more power than other devices simply because it inputs rapid multiple thrusts. Power is the rate at which energy is transformed, which takes the time factor into consideration. (Power = force x distance per unit of time). Comparatively speaking, when set at the same thrust as another instrument, the multi-thrust ArthroStim™ Instrument will deliver far more power in the same amount of time.

An ArthroStim™ Instrument will deliver continuous proprioceptive dynamic energy for as long as the trigger is pulled. However, a momentary half-second pull of the trigger is usually optimum. Often, after a chiropractor experiences the effectiveness of a short burst of rapid thrusts, he or she tends to assume that excessive rapid thrusting of more than one second continuous will prove even more successful. Rapid thrusting beyond the point of diminishing return (point at which more energy is being put in than good is coming of it) may be counterproductive. One short half-second burst is usually more effective than a long burst, yet occasionally a few short bursts at various vectors will be necessary. Training, technique, and experience determine the number of thrusts to induce. The momentary rapid thrusting cumulatively achieves optimum joint displacement with maximum patient comfort.
